- B6 - Dynastic - Klash double up OPA 8-4 to claim another B Championship
- Playing their most complete game of the season, Klash-B fended off a brief OPA rally and skated away with the B leaue crown 8-4. This championship is the fourth in five B League seasons for the Klash and their sixth overall.
During the season and playoffs, the Klash played alternately well on offense, defense, and between the pipes. They survived two bumpy wins to get this title match but left no doubt in their multiple faceted effort. They staked a 3-0 lead, absorbed an OPA three goal rally, and then throttled down to a convincing win. Fittingly, the Klash were led by their Swiss Army knife, B League MVP Kyle Sidle. Sidle, equally capable north and south, scored perhaps the two most important Klash playoff goals. Trailing 5-2 late second in the Klash's first playoff game, Sidle scored twice in 15 seconds to reverse momentum and spur the subsequent playoff run.

- C7 - Quick Strike - Snipes stun Squirrels with four in first five, skate to 8-2 C championship win
- Scoring early and scoring often, Snipes jumped the Squirrels in the first five minutes to seize the C League championship 8-2. With the victory, Snipes earned their second LI Cup and repeated as Fall C league champs.
Kicking off the early scoring barrage was C League MVP Steven Wagner. Wagner scored the first goal, assisted on the second, and added another goal and two helpers later in the game. Snipes scored four times in the first five minutes and stretched the lead to eight by the middle of second. In total, Wagner scored six times in the Snipes hard fought playoff run.

- Championship Game
- D6 - Viva Le Pepe - Euro Stars blank LI-Stars 2-0 to win D1 championship
- Showing off their versatility, the Euro Stars played smothering defense to stonewall the LI-Stars 2-0 for the D1 championship. The victory is the Euro Stars second Fall D1 title in three years and third LI Cup in the organization's history.
After a scoreless first period, the Euro Stars got all the offense they needed when Kevin Azevedo scored just 23 seconds into the middle stanza. The Euro Stars demonstrated during the regular season that they can play any style, any pace. They led the division in both scoring and goals against. In this title tilt, they went defensive. Backboning the lockdown D was the stellar play of D1 League MVP Domingos Magalhaes. The big netminder was both steady and steadfast in yielding just three goals in the Euro Stars three game playoff run.

- DD7 - Matty Ice - Mayhem-D scores late to nose out Minutemen 5-3 in D2 championship nailbiter
- In a close, tense match befitting a winner-take-all, Mayhem-D scored two late goals to put away the Minutemen 5-3 and grab the D2 League championshp. The D2 title is Mayhem-D's first Long Island Cup championship.
The previous two playoff match-ups between these two teams saw the squads splitting overtime wins. Thus, it was not surprising to see the Minutemen scrape three times to erase one goal Mayhem leads. In the final period, tied at three, each team had seemingly go-ahead chances only to be thwarted by stellar saves. Mayhem finally prevailed with just two remaining when D2 League MVP Matt Ronan potted the titlewinner. Ronan led Mayhem's playoff scoring with three goals and seven assists.

- E4 - Five Alarm Parade- MFD extinguishes Redrum 5-3 to claim E1 championship
- Bending but not breaking, MFD held off a determined Redrum squad 5-3 to earn the E1 championship. MFD scored twice in the last five minutes to clinch the win. The E1 title is MFD's first Hockey Long Island Cup in the organization's three years.
Leading 3-0 and controlling the pace of play, MFD seemed to have the match firmly in hand. Inexplicably, late in the second period, the game flipped. Redrum scored twice in four minutes and subsequently tied it up middle of the third. Reeling, MFD turned to E1 League MVP Eric Conway. Conway, who previously scored on a shorthanded penalty shot, rang the bell with a right center slot blister off the far post with under five to go. Conway scored in each of MFD's three playoff games tallying five goals overall.

- EE4 - 2G1-LI Cup - 2G1P shortcircuits High Voltage 7-4 for E2 title
- Fending off a scrappy High Voltage squad, 2G1P finally pulled away in the third to capture the E2 championship. 2G1p scored twice in the final four minutes to clinch the title. The victory is 2G1P's first LI Cup.
After spotting an early High Voltage goal, 2G1P scored three unanswered and the match started to resemble the previous match-up between these two teams. High Voltage, however, said no. With spunk and vigor, they scored late first and early second to tie it at 3-3. 2G1P answered right back with two but Voltage closed again with a late second tally. It wasn't until E2 League MVP Matt Coon. potted the critical insurance biscuit that 2G1p sealed the championship. Coon's goal completed his second playoff hat trick and padded his playoff scoring total to eight overall.

- 3D7 - The Scorpion Kings - Scorpions wrest 3D crown from Court Officers 3-2 in overtime
- In dramatic, gripping fashion, the Scorpions scored suddenly in overtime in the decisive match against the Court Officers to prevail 3-2 and grab the 3D Championship. The 3D title is the Scorpions fourth Long Island Cup and first season championship in three years.
In this see-saw affair, each team came from behind to settle into an early third period 2-2 tie. In the two previous playoff games between these teams, a close game broke open to one side late in the third. Not this time. Snappy up and down play produced near-misses alternating with shutdown netminding but all failed to yield a champ. Overtime was needed and five minutes in the Scorpions danced. As often the case, a slippery puck found a fortuitous stick. Off a seemingly innocuous break in, an unintended redirect squirted to a well positioned 3D League MVP Andy Pennecke who buried a layup titlewinner. Pennecke scored four times in the Scorpions playoff drive and assisted on an earlier playoff overtime winner.

- 3E6 - Belly-to-Belly - Bladerunners round up Banshees 5-1 to again wear 3E crown
- Jumping in front early, the Bladerunners stymied the Banshees en route to a 5-1 victory in the 3E Championship match. This latest title defends the Bladerunners' Spring '12 3E championship and is the organization's fifth Long Island Cup.
The Bladerunners scored three times in the first period which proved all they needed. The Bladerunners, renowned for a deliberate, defensive attack, limited the action in their defensive zone through out the game. They held the Banshees to four first period shots and just 17 shots overall. Bolstering the atypical spry Bladerunner offense this postseason was the sniper fire provided by 3E League MVP Pete Sheridan. Sheridan scored in the championship game and led the Bladerunners in the playoffs with four goals in total.
